Prepare for your Initial Visit
What to Bring
- A valid photo ID
- Your insurance card(s), for your primary and any secondary insurance
- A valid credit card to leave securely on file
- A complete list of your current medications, or the medications themselves
- Any medical records you may have, including prior testing, imaging studies, labs or office notes**
- Prior to your visit, please visit your patient portal to view and electronically sign our required practice forms and enter your health history information through the portal
** If you are in need of assistance relating to the release of medical records from another provider, please contact our office. We are happy to help you.
