How to check in for an appointment and sign forms

Step 1: Log into your portal and click the blue button Check In

Step 2: Review your personal information. If it needs to be updated, click Update.

Step 3: Review your preferred pharmacy. You may add up to 5 pharmacies by clicking Add Pharmacy.

Step 4: Read through the office forms. Once complete, type your name in the box and click Sign.

Step 5: Repeat until all forms have been completed.

Step 6: Review your health history. If there are no updates needed, click No Changes.
If an update is needed, click Update.

Step 7: Click Complete at the bottom of the screen.

You are all done!
