Prepare for your Initial Visit
What to Bring
We strongly recommend that prior to your visit, you log into your patient portal and input all of your health history and detailed medication list.
Please bring the following items with you to your initial visit OR you can text them to us securely in advance of your visit:
- A valid photo ID
- Your insurance card(s), for your primary and any secondary insurance
- A valid credit card to leave securely on file
- A complete list of your current medications, or the medications themselves
- Any medical records you may have, including prior testing, imaging studies, labs or office notes**
Mobile Check In
For a faster and more convenient check-in experience, you can use the APPatient™ app to complete your check-in up to 24 hours before your visit. By using the APPatient app, you’ll minimize wait times and streamline your visit. If you need help in setting up or using the app, our team is happy to assist!
Copays are required prior to services being rendered. For patients who pay their copay through the APPatient app, we ask that they do not also pay a copay at the office, duplicating their payments. Thank you.
